2015 Season Coach's Affidavits

Coach's Affidavits are just as important, if not more so than the Athlete's Rosters.

It is our job to ensure that our student athletes are surrounded by coaches who have passed all of the required background checks, and have completed all of the necessary classes as set forth by the Arch Diocese of Philadelphia.

The Coach's Affidavit MUST be filled out completely, and any person who spends more than 1 hour a week with student athletes MUST be listed on the Affidavit.  As coaches, you are ultimately responsible for the athletes, and the adults that you allow to work with them.

The following information must be listed for each individual Coach:

1. Coach's Name
2. Coach's Parish
3. Date of Coach's Orientation Attended. (This MUST be a date.  Simply writing completed in the box is not acceptable)
4. Date of Mandated Reporter Training Attended.  (This MUST be a date.  Simply writing completed in the box is not acceptable)
5. Date of Safe Environment Session Attended.  (This MUST be a date.  Simply writing completed in the box is not acceptable)
1. As of August 1, 2011 the Technology Addendum was included in the Safe Environment session. 
6. PA State Police Clearance on file in parish office?  (Yes is the only acceptable answer for this question.  Date is not necessary)
7. PA Child Abuse Clearance on file in parish office?  (Yes is the only acceptable answer for this question.  Date is not necessary)

As a head coach The Regional & Area Commissioners trust that you have verified each of the above certificates to ensure that they were completed and are still valid.  Do not blindly sign a Coach's Affidavit as if there is an issue down the road it will jeopardize all of our standings in the Arch Diocese.

This affidavit must be submitted on April 6th along with the Student Athlete Rosters.
